Topic: Chapter 4 - Introducing an Electronic Prescription to an outpatient multispeciality clinic in Dubai

Order Description Chapter 4: The Evaluation Chapter The Dissertation title: implementation of using E-prescription to a multi-speciality clinic • In Chapter 3 we’ve discussed the full implementation of e-prescription to a multi-speciality clinic using the HSE change Model. • In this chapter we’ll use qualitative and quantitative evaluation methods • It should be associated to literature review • It should be linked directly to the stated objectives in Chapter 1 • Collect Data Analyse Graphically illustrate analysis Kindly use the following headings: 4.1 Introduction: What’s healthcare evaluation & why it’s important. 4.2 Evaluation Models(Don’t Exceed 600 Words Limit) :briefly write about some evaluation models used in healthcare (3 to 4 )(for example: CIPP Evaluation Model, “Box” model for evaluation design, Lazenbatt Evaluation Model, The four level Kirkpatrick Model). At the end of this part introduce next part in which the author will evaluate each objective discussed on chapter one: 4.3 Evaluation of Current Implementation Objectives: 4.3.1 Evaluation of 1st Objective: • By 30th March 2016, the physicians will meet (hit) 95% compliance rate against new system guidelines. You can start writing on physician’s culture & behavior when it comes to change in healthcare; some resistance was there from two old school physicians, this behavior totally changed after a month of full implementation because of management full support. This objective will be measured using auditing as below: Nominator: total number e-prescriptions/ month : it was collected from the clinic software Dominator: total number of manual prescriptions/month: It’s a two floor clinic, two manual prescriptions were left at each floor nurse station under the responsibility of the nurse in charge. Every time a doctor has to write a manual prescription he/she has to request it from the nurse station, the reason should be documented on each prescription. Findings: (write about the following findigs also you can “if found” relate your findings with an evidence based studies”) Month Total Prescriptions E-prescriptions Manual Prescriptions Reason January 1420 1055 365 333 due technical issues/32 due out of stock medications February 1623 1599 24 4 due technical issues/20 due out of stock medications March 1485 1450 35 2 due technical issues/33 due out of stock medications April 1722 1697 25 2 due technical issues/23 due out of stock medications This evaluation will continue for future months for organization reference 4.3.2 Evaluation of 2nd Objective: • By 30th March 2016, the total number of dispensing near miss incidents will be decreased by 99%. This could be measured as below Nominator: total number of near miss incident Dominator: total number of suspected and communicated near miss prescription We have an online incident reporting system all the data will be entered by the reporting staff, sent to our quality department, analyzed every month for further actions to be made. For the past 3 months we’ve noticed a drop in the incidents were near miss occurred due poor handwritten prescription, it gave us the impression that it was a direct result of implementing e-prescription in our clinic. (you can support your result with similar studies shows almost the same results) This evaluation will continue for future months for organization reference 4.3.3 Evaluation of 3rd Objective: • Pharmacy visit experience’ patient satisfaction will be maintained 90% and above by 30th March 2016. Or, patient stratification post pharmacy visit will be maintained 90% or above by 30th March 2016. As Patient satisfaction depends on: • Increase waiting time to be checked by pharmacist • Increase pharmacy visit time due to spending long time to clarify doctors order(you can keep a target of 10 minutes) of total visit • Dispensing wrong medications • Feeling unsafe and lose patients’ trust in their doctors A patient satisfaction survey was designed using the SurveyMonkey tool (Surveymonkey.com, 2016) Surveymonkey.com. (2016). SurveyMonkey: Free online survey software & questionnaire tool. [online] Available at: https://www.surveymonkey.com [Accessed 10 Jan. 2016]. (Survey questionnaire template & results will be uploaded for your reference). Same like previous This evaluation will continue for future months for organization reference 4.3.4 Evaluation of 4th Objective: • By 30th March 2016, all pharmacists will be able to demonstrate 100% compliance rate to new system protocol. This can be measured by random auditing, observations, …etc An announcement was made for all pharmacists not to accept any manual prescriptions unless signed by the head nurse of each floor, patients with unsigned manual prescription had to collect their medications from a community pharmacy. Regular audits were made showed that pharmacy staff were compliance not to accept any unsigned manual prescription Same like previous It’s good to show that this analysis will continue for future reference 4.4 Conclusions: What was learned from this chapter & signpost the next (Discussion Chapter).
