Case Study
Mr. Joseph is a 68-year-old male hospitalized for shortness of breath and chest pain. He was referred to the hospital by his primary care physician with complaints of shortness of breath climbing the stairs in his home and chest pain and fatigue after walking 2 blocks. Mr. Joseph also reports syncopal episodes after exertion.
Mr. Joseph is married and has two grown children. He enjoys walking his dog, gardening, and woodworking in his home woodshop. He enjoys gardening and reports he eats most of the vegetables he grows and is learning make “healthy” smoothies for his breakfast with kale and other greens. Mr. Joseph is a retired carpenter and volunteers his time making ramps for hospice patient’s homes. He lives in a private home with one flight of stairs to the bedrooms.
Mr. Joseph is a non-smoker and enjoys a cold beer after gardening twice a week.

Diagnosis: Aortic Stenosis
Treatment Plan: Mr. Joseph had a surgical consult and an elective surgery for aortic valve repair is scheduled in 4 weeks.
Discharge Medications
Digoxin (Lanoxin) 0.125 mg PO daily
Diltiazem (Cardizem) 30 mg PO two times per day
Amiodarone HcL (Cardarone) 200 mg PO daily
Warfarin sodium 5 mg po daily
