(This document from the CDC lists commonly used community assessment models: https://www.cdc.gov/stltpublichealth/cha/assessment.html) What are some benefits of using a specific model to assess a community? What agencies should be included with the assessment? What is a reasonable time frame in which to complete a community wide assessment? What is the role of the public nurse in implementation of the chosen model?

 

Problems facing individuals leaving prison and reentering your community.

 

 

 

Imagine you are a local community leader and are bothered by problems facing individuals leaving prison and reentering your community. You do not feel your state or local community treats these individuals ethically and have decided to write a letter to your congressman requesting their support for changes that would more ethically support those individuals.

Select 1 common problem for justice-involved individuals from the following list:

Overcoming stigma of being incarcerated or being on probation
Seeking employment
Housing
Community support
Identify your community and the services in your community.
Describe your selected problem for your community.
Discuss how ethics should come into play when designing solutions for inmate issues.

A crisis in the community

 

 

 

The crises you select may be crises you personally experienced, learned about through this course, or heard about on the news, past or present. The crisis should be discussed from an academic lens and written in scholarly voice.

Then, for each crisis, complete the following:

Describe the crisis, including: the event/situation, implications, responders, victims, survivors, and perpetrators.
Describe the forms of critical incident assessment you might use.
Explain the potential short-term and long-term effects that might be experienced by the people impacted by the crisis.

Research agencies in the community and create a resource manual that will help you with clients in the future. Packet should include agency name, address, phone number, services provided and age range. You should include at least 20 agencies available in the Treasure Coast (Stuart, FL, Port St. Lucie, FL., Fort Pierce, FL)

Volunteers and political officials in local communities often campaign to improve conditions and provide services to increase the well-being of individuals and families living in those communities. If you are the parent of young children, you might focus on improving the local school or creating safe places where children can play. If you are an individual in later adulthood or a caregiver for an individual in later adulthood, what community resources might be important to you? For this week’s Assignment, you evaluate the resources that your local community provides for its older members.

To prepare for this Assignment, research the resources available in your local community to support the issues and concerns of the older population. Note any gaps in these services and consider what improvements might be made to existing services as well as what services should be added.

 

Submit a 2- to 4-page paper that includes the following:

A description of the services in your local community that support individuals in later adulthood
An evaluation of the effectiveness of the services you identified
A description of service gaps you identified
An explanation of how to improve existing services
