Clinical site was hospice located in Milpitas, CA
The first clinical reflection can focus on how we attended IDG meetings (which took about 3 hours each morning). These meetings are hospice meetings that
provide a chance for an interdisciplinary group to meet and discuss a patient’s plan of care and properly negotiate involvement from the patient themselves,
their family, and various caregivers. The doctor, chaplain, nurse, social worker attended these meetings.
The second reflection can focus on post admit calls. These were calls we conducted to survey families regarding initial care and addressing an questions
and concerns for new hospice patients.
